Below is a blank RFM (Request for Modification) application form. Fill it out completely and return with a marked drawing to the Board. Use one of the printable forms below as noted in the application.
The following are printable forms that will assist you in the planning of landscape modifications to the rear of your units. Please be advised that the Board will not approve any modification that exceeds these limits. The forms are generic and are available in 2 variations as noted below.
